Robert Conti:

"The Chord Melody Assembly Line" (Book and 4 hours DVD)

and

"The Formula" (Book and 6 hours DVD). I'd recomend both.

"The Assembly Line" teaches you the basics, "The Formula" teaches you all you need to know about re-harmonising.

this may be a bit late but I ran into this lesson online and thought it would be able to give you a few good pointers on Chord melody check it out http://www.jazzguitarlessons.net/chord-melody.html This is one of my favorite sites overall you'll find tons of usefull information. hope it helps.
