Yes, it comes with everything you need. Double sided adhesive holds the jack to the tailpiece and putty secures the pickup to the top. It's all removable and non-permanent.
The only mod I did, I got from Jim (Jkaz), a good one - adhesive-backed velcro on the tailpiece, and on the jack. Sticks well, but nothing permanent. I found that the adhesive that is used on the rubber foot of the jack wasn't enough, on my guitar - the jack kept falling loose during our set.

I have been very pleased with the Ischell and use it regularly on gigs with a fairly loud quintet (drums, sax, trumpet, and bass). It sounds great on the ALD La Brune I'm using now and it was just as good with the Dupont MDC-50e I used before. I have used it on two Epiphone archtops with excellent results as well. It resists feedback very well, on the few occasions where there was any it's always been that I needed to make the seal between pickup and top a little better (which takes seconds).

I think I just got it as part of an Amazon purchase, but I forget, to be honest. It's really common stuff. I think most fabric stores carry it?
Edit: Actually, this is the stuff I got, I think. I just cut it to size.
