Largest I used was a wegen 5mm. I played rhythm for years with that thing before I bothered starting to learn lead. The 5mm had a fairly round tip and so it swished over the strings nicely, never catching.
A pick that big is hopeless for any lead playing, much too fat. So later I changed to a 3.5mm, and when my technique improved further I found even that was too fat. 1.5mm-2mm is about right now.

I used to use a Wegen 7 that I rounded to the Button profile. Loved the feel for both rhythm and lead. I would still happily use it if it wasn't so damn noisy.
Depends on the player; I know that Reinier Voet plays with a 4 or 5mm, and Fapy and Rino use something around 5mm. I find that to be the ideal thickness ( provided the tip is nicely beveled and rounded) to get a great tone for lead as well as rhythm. As far as largest diameter, 3.5 x 3 cm is the size of the pick i use most of the time, anything larger is hard for me to keep a grip on.
