I am not sure if you understood my question - I am wondering if the 4mm is measured from the wood of the fingerboard to the string, or from the top of the actual fret wire to the string.

From the top of the fret. I discovered this the following way. Three years ago I was using a Favino with an adjustable bridge and was visiting Ninine. He looked at my guitar and commented on the action, and I told him to "make it right". He jacked it up and thats where he placed it. Last year, I got a guitar which was set-up by Moreno, and it was set to the same place.
